Better health outcomes begin with better coordination. The Emergency Department Care Coordination (EDCC) program equips providers, care teams and policymakers with near real-time health information, improving patient transitions, guiding smarter decisions and advancing statewide health outcomes.
Connecting Care Across Virginia
The EDCC, administered by VHI, is Virginia’s statewide system for turning isolated encounters into coordinated care. The program combines real-time notifications, a secure online portal and shared patient insights to connect hospitals, health plans, providers and community partners. By ensuring the right people have access to the right information at the right time, the EDCC reduces avoidable hospital visits, improves follow-up care and supports smoother and stronger health journeys for Virginians.
Information Shared by Hospitals
All Virginia hospitals are required to submit care coordination information such as hospital encounters, care plans, lab results, images and discharge information to support clinical and care management personnel.
Real-Time Notifications
Information is shared among providers in near real-time via customized alerts and notifications that identify patient-specific risks.
Tools for Providers and Care Teams
Healthcare providers, care team members and insurance carriers can gain access to a web-based portal for viewing and contributing information to support collaboration.
Emergency Department Access
Emergency departments receive an array of customized alerts and summary reports with key information on patients’ visit histories at acute and post-acute facilities.
