• Complete all required fields including identifying information
• Agree to the Terms and Conditions
• Upload at least one healthcare document

Once you upload your Advance Care Planning Documents to the U.S. Living Will Registry database, they are secure and will not be shared or sold to any third parties as U.S Living Wills Registry voluntarily complies with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
