“We had a client beginning to experience cognitive and physical decline at an alarming rate while living in an independent senior community,” Shakeisha began, reflecting on the case that proved the critical difference timely information can make in care.
The Prince William Community Services Board Case Manager knew she needed trusted data to act quickly. “[The client’s] housing was in jeopardy after repeated falls caught on security cameras led to an Adult Protective Services (APS) report,” Shakeisha continued, “Although she had a valid Uniform Assessment Instrument (UAI) on file, documentation was needed to support a reassessment.”
Up-To-Date Data Means Stronger Outcomes
That’s when Shakeisha turned to VHI’s EDCC program for the right information. By utilizing EDCC reports and inpatient documentation, she gained a comprehensive view of her client’s care history.
“I was able to track her hospital stays, diagnosis codes and discharge recommendations, which she often discarded. This allowed me to work with the Prince William County Health Department to rescreen her immediately. The client was approved for in-home care and received services until she passed away at home as she desired.”
For Shakeisha, the EDCC program offers more than a data tool; It’s a bridge between information and action. “The EDCC has greatly enhanced my ability to advocate for clients, coordinate care and help them achieve their health goals.”
