Facility Comments: PACE stands for Program of All-Inclusive Care for the Elderly. We are a voluntary Adult Day program whose primary purpose is to assist the frail elderly to remain as independent as possible. The program is designed to keep participants living in their own homes in their communities. We are located at 5788 Churchland Blvd. Portsmouth VA. The services we offer are adult day programming, specialized activities, nursing care, social work, physical, speech, and occupational therapy, nutritional assistance, home care, prescriptions, medical equipment as well as transportation.
