Facility Comments: Sentara Home Care Services provides home health, hospice, pharmacy/infusion, and Philips Lifeline Personal Emergency Response System. We work closely with physicians to coordinate the multiple therapies and services each patient may require – all in the comfort of home. Services provided include skilled nursing (from wound care to chronic disease management to rehabilitative therapies), as well as Telehealth remote monitoring to measure patients’ vital signs for more frequent observation. We also offer Hospice care for patients with life limiting illnesses – providing comfort, support, and dignity to patients and their families. Home Pharmacy/Infusion is provided for acute and chronic illnesses, including specialty medication therapies and ambulatory infusion pumps.
